This health and wellbeing at work assessment is easy to use with your workforce. It contains 12 questions and each respondent needs to think about what has been happening at work over the past couple of weeks.
The scoring method is that anyone who circles either of the two right hand columns from Qs 1-10 and the left two columns for Qs 11 and 12 is showing some kind of distress that needs further investigation.
